PERSONALIZED GERIATRIC ASSESSMENT
The initial 2 to 4 hour face-to-face visit with client & family/caregiver is to develop rapport, and assess the following:
Current Functional & Cognitive Status
Environmental (home) safety & needs
Depression and/or Anxiety
Medical conditions & history
Current Medications
Health Care Power of Attorney (HCPOA)
Advance Care Directives &/or planning
Durable Power of Attorney (DPOA)
Personal History Review
Caregiver Stress
Consent (client &/or HCPOA) for Sandi to accompany client to medical appointments & obtain/review medical record of client (if desired).
Review Contract for services
FEES
Initial Assessment:
Fees for Initial assessment run from $300-$450, depending on the complexity of situation
A substantial assessment report (included in above fee) will be provided to client and/or HCPOA/ DPOA via email, within 72 hours of the conclusion of the Intake/Assessment visit.
Note: for some geriatric patients, a two to four hour visit is too long. In this case, the Initial Geriatric Assessment will be scheduled over 2 different dates. A packet of information will be provided to the client/caregiver prior to the Initial Assessment.
